Hazel Ann Stein, 55, of Bartonville passed away Monday, March 9, 2020 at OSF St. Francis Medical Center in Peoria.
She was born August 25, 1964 in Peoria to John M. Sr. and Helen Vance Stein.
She was preceded in death by her father and brother, John M. Stein Jr.
She is survived by her daughter, Alexis M. Stein; mother, Helen Choate; brother, James E. Stein, sister, Theresa S. Stein and multiple nieces, nephews and cousins.
Cremation has been accorded and no service will be held at this time.
